Pamela Nascimento, 27, is believed to have suffered a perforated kidney at the private clinic.
But chiefs at Green Hill Hospital in Sao Paolo, Brazil, claim she went into cardiac arrest during the procedure.
It was the third time the model, who appeared as a glamorous co-host on several prime time game shows, had a stomach-flattening op.
The doctor who performed the operation, Julio Yoshimura, faces possible charges of manslaughter.
Pamela died on October 19 but her death was only made public this week.
